Managing your daily self-care for diabetes can be challenging. At the Harry Hay Wolf, Jr. Diabetes Center, our nutrition and diabetes care professionals are here to help. Our certified diabetes educators (CDEs, health care professionals trained in managing all types of diabetes) provide education and support services for patients and their families. We help you understand and manage your disease for better health and quality of life.

 

Diabetes Education at Presence Saint Francis Hospital: Why Choose Us?

Our nutrition and diabetes education program offers:

  • National recognition for excellence: Our program has earned accreditation from the American Diabetes Association (ADA) and the American Association of Diabetes Educators (AADE). That means you can expect the best quality services from specialists who meet and exceed the National Standards for Diabetes Self-Management Education.
  • Highly trained and certified specialists with decades of experience: Since 1995, both of our CDEs have specialized in nutrition and diabetes education. With degrees in nursing and certifications in dietetics and nutrition, they stay current with the latest developments in diabetes management.
  • Comprehensive services for all types of diabetes: Our CDEs help adolescents through adults with prediabetes, type 1 and type 2 diabetes and gestational diabetes.
  • Coordinated care to support your overall health: We work with your referring physician to understand your individual needs and develop recommendations. If you need care for diabetes-related conditions, we work with other departments at Presence Saint Francis including cardiovascular (heart) care and wound care.
  • Expertise for better patient outcomes: As a busy diabetes education center, we treated nearly 900 patients in 2015. By seeing a high volume of patients, we have built our expertise in helping patients with a wide range of needs.

 

Our Services for Nutrition and Diabetes Education

We offer solutions to help you control diabetes and live well. We work with you to develop a self-management plan for everything from taking medication and monitoring your condition at home to eating healthy and staying active. You’ll learn problem-solving skills to help you cope with the unexpected, in a daily routine that helps reduce your risk of complications.

Medicare and most health insurance plans cover diabetes education as part of your diabetes care. Check with your health insurance company to find out what services your plan covers.

We offer the following services:

Diabetes Education

  • Diabetes self-management education program for outpatients
    • Group education classes twice weekly to introduce you to healthy eating at home and out, getting physical activity, taking medication properly and more
    • Individual sessions, if preferred, by appointment
    • Annual refresher class for follow-up
  • Individual counseling for women with gestational diabetes (high blood glucose (sugar) levels during pregnancy)
  • Individual planning for exercise and weight management
  • Nutritional counseling and education on healthy food choices
  • Diabetes education for inpatients and training for nurses in Presence Saint Francis departments to help patients transition home

 Community Outreach for Diabetes Education

  • Free, monthly community diabetes support groups, open to everyone, on topics including foot care, neuropathy (nerve damage in the feet or hands), heart health and new medications
  • Annual health fair with services including:
    • Screenings for blood sugar and blood pressure
    • Body mass index (BMI), a measure of body fat based on height and weight
    • Foot care such as daily foot checks, an annual foot exam and proper nail care to prevent complications
    • Education on nutrition, self-management, wound care and prevention of diabetes-related complications

 Training and Self-Care

  • Self-administration training for patients, their families and caregivers to give insulin at home, including how to:
    • Use an insulin pump
    • Inject insulin by syringe or insulin pen
  • How to monitor your blood glucose (sugar) at home
  • How to handle medical emergencies such as low or high blood sugar
  • How to prevent and manage long-term complications such as eye problems, heart disease, kidney disease, neuropathy and stroke
  • Self-care specifically for diabetes patients, including foot care, skin care and when to have eye, dental and other exams

 

Our registered dietitian offers nutrition counseling for people who are having weight-loss surgery at Presence Saint Joseph Hospital. She meets with you for up to 6 sessions before your surgery to develop a customized plan for lifelong healthy eating.
