Managing diabetes is the most important aspect of living with the disease.  At Presence Mercy Medical Center - Center for Diabetic Wellness, we offer a unique, state-of-the-art program that can help you control your diabetes and live the most productive life yet.

The Center for Diabetic Wellness (CDW) has been awarded Education Recognition Program Certificate status by the prestigious American Diabetes Association (ADA) since 2005. 

By certifying the Center for Diabetic Wellness, ADA recognizes that our program offers high quality education, which is an essential component of effective diabetes treatment.

The Association's Education Recognition Certificate assures that our educational program meets the National Standards for Diabetes Self-Management Education. 

Our highly qualified team of Certified Diabetes Educators works closely with you and your healthcare provider to develop an individualized plan of care to suit your needs. 

Over 60% of our program graduates have achieved control of their diabetes upon completion of our program. 

 

Our Approach 

Our comprehensive program offers outpatient services for individuals with Type 1, Type 2 and Gestational Diabetes. 

Our Life with Diabetes Program consists of an initial one-hour individual session with a nurse educator and 8 hours of group class sessions

Finger stick test

During your initial session, our nurse educators review your diabetes history, assess your learning needs, establish blood glucose target ranges, and individualize your goals for the program. 

During your group class sessions, our diabetes care team (pharmacists, nurses, dietitians, counselors) will work with you to help you meet your goals. 

You will also have two nutrition sessions with our registered dietitian to develop an individualized meal plan. 

Bilingual staff are available at all times.
