What is Medicare Value Partners?
Medicare Value Partners (MVP) is a Medicare Shared Savings Program (MSSP) Accountable Care Organization (ACO). The goal of MVP is to help your practice better coordinate care for your Medicare fee-for-service patients. Patient Notification Process
MVP will provide participating ACO facilities with a sign that is to be displayed at all times. This sign is the only written notification all ACO providers are required to display and notifies the patients who are attributed to MVP of their primary care physician’s participation in the ACO. How Can Care Coordinators Help Patients and Physicians?
Care Coordinators have specialty training in complex care coordination and help counsel patients to understand their specific health care needs. They help patients become active participants in their own care. They can help you manage patients who have behavioral issues, chronic illnesses, or haven’t been back to the office for a long period of time. A partnership with a Care Coordinator helps you meet goals for care quality and efficiency while increasing office visits and decreasing avoidable inpatient hospitalizations. The Care Coordination program is available to all physicians who participate in MVP free of charge.
