Presence Health

Through our Mission, Presence Health is called to improve the health and well-being of the communities we serve. As a Catholic institution and a not-for-profit organization, supporting the community partners is part of our social tradition. We strive to engage with those partners in the pursuit of wellness. To that end, we support events, programs and initiatives that seek to bring healing and hope to our neighbors.

Here you will find our priorities for community support as well as a form to submit a request for funding or in-kind donations. First, we recommend familiarizing yourself with our Community Health Needs Assessments. Each ministry completes a thorough report on the needs of its community and these reports help guide us in our support and sponsorship efforts.

As a System we choose to focus our Community Health efforts on four specific areas, however, each ministry has additional implementation strategies, which could align with your request for support. The System four focus areas are:

  • Access to Care
  • Chronic Disease
  • Mental Health
  • Social Determinants of Health

Eligibility for Support
Every year Presence Health supports events, programs and initiatives throughout our service areas. Requests for support should fall into at least one of the following categories—

  • Supports Presence Health's Mission and Vision
  • Aligns with a ministry’s CHNA Implementation Plan
  • Falls within Presence Health’s targeted markets and service areas
  • Reaches diverse populations
  • Focuses on health and wellness education
  • Supports disease prevention
  • Supports healthy, active lifestyles
  • Requests for Presence Health's business or clinical expertise
  • Requests for medical support, health fair participation, or health screenings
  • Requests for volunteer efforts from Presence Health associates

We are unable to support requests for support of political candidates or organizations.
